New Patient Registration Form
How to Register
Surgery Registration Times : Monday – Friday 10.30am – 12.30pm and 2.00pm – 6.00pm.
To register with our practice please complete the attached GMS1 form and Health Questionnaire. We will require two forms of identification photo ID such as Passport/Driving License along with proof of address such as a utility bill/ statement dated within the last three months.
Please send the completed forms along with the supporting documents to our email address: breccg.chalkhillfamilypractice@nhs.net.
Until we have seen your identification you will not be registered at the practice.
